Job Summary:

To assist in the performance of quality, caring clinical dentistry; to become adept at assigned clinical procedures; to aid the doctor smoothly and unobtrusively and aid in keeping patient flow steady throughout the day. Perform clinical and technical procedures under the super¬vision of a licensed dentist. The DA gets the patient ready, prepares the operatory and equipment and assists the doctor during examinations and treatment. Must have the ability to multi-task and handle multiple patients and projects efficiently and thoroughly. The applicant maintains awareness of comfort and safety needs of patients and anticipates the potential needs of the doctor and patient.

The applicant should be able to competently perform the clinical and technical dental procedures required of a Certified Dental Assistant. Applicant is able to take quality x-rays, including FMX, Panoramic and anterior PAs. Knows how to take blood pressure, attach EKG leads to patients and understand readings of monitors. Thorough understanding of how to prevent cross contamination and keeps informed and updated on OSHA regulations. The applicant will have to physically assist drowsy patients after anesthesia. May have to lift up to 25 pounds, roll a 90-pound anesthesia machine and a full 35-gallon drum of expended chemicals on wheels from one area to another.

MINIMUM Requirements, Certificates, Licenses:

Fluent in English and fluency in Spanish strongly desired

High school diploma or equivalent

Dental Assistant Certificate

X-ray License

CPR Certificate

At least 3 years of actual dental assisting experience

Surgical assisting experience required (oral-maxillofacial)

Knowledge of Dentrix (G2)

Knowledge of CDT dental coding

Familiarity with the following machines required: Autoclave, model trimmer, X-ray machine – Ceph, Panoramic X-ray, X-ray processor, X-ray duplicator, ultrasonic, O2 and N2O, laser, Intra Oral camera, Laser, Vacu-press, Paché gun, KCP 2000, Whip-Mix and hand pieces.

Homes for the Homeless Transitional Housing Program

LOCATION: Northern Virginia

REPORTS TO: Executive Director, Americans Helping Americans

JOB SUMMARY: Responsible for overall planning, administration, supervision and evaluation of transitional housing program in which case management services are provided by 14 human service or ?Sponsoring? Agencies (SAs) in 45 scattered-site residential properties. Develop and administer program policies and procedures. Provide direct supervision of case management services for Safe Places (domestic violence program), serving as a Sponsoring Agency for 10 of the 45 housing units. Conduct staff supervision, program development, grant writing, quality assurance and program compliance.

DUTIES/RESPONSIBILITIES:

? Coordinate and meet with individual Sponsoring Agencies to monitor case management services.

? Supervise H4H Property Manager, Safe Places Case Manager and volunteers

? Evaluate prospective program referrals, determining preliminary eligibility

? Conduct intake, recertification and exit interviews.

? Review budgets, service plans and make recommendations.

? Maintain regular and on-going contact with SA Case Managers and clients regarding service plans.

? Ensure H4H staff participation as a member of case management teams to develop client goals and monitor client progress.

? Conduct team interventions to support Case Managers when residents are in serious non-compliance with their service plans, engaging in behaviors destructive to their goals or facing formidable difficulties not readily addressed by Case Managers.

? Conduct program orientations for new Case Managers and Sponsoring Agencies.

? Conduct Sponsoring Agencies meetings to reinforce program guidelines and provide training.

? Ensure contract compliance with Funders.

? Prepare new and re-newal grant applications (e.g., HUD, Virginia DHCD, Fairfax County, etc.)

? Ensure compliance with Fair Housing and Landlord/Tenant laws.

? Develop and update housing resources.

? Provide housing counseling resources to SA and residents, ensuring that all residents are seeking permanent housing.

? Attend housing/human services conferences, seminars, workshops and meetings for on-going training, community involvement/education, advocacy, countywide needs assessment and grant writing.

? Troubleshoot with the Property Manager on matters that can affect our community standing.

? Gather and prepare monthly, quarterly and annual reports for Share Shelter, CCFP and HUD.

? Prepare monthly and quarterly program reports.

? Conduct on-going evaluation to strengthen existing program needs developing action plan to address those needs (e.g., updating or developing new policies, program documents and addressing issues of Sponsoring Agency insufficient services, failure to follow guidelines or inadequate reporting Agencies.

? Prepare and/or review weekly payment requests as needed.

? Monitor monthly program expenses, including utility usage.

? Monitor and assist Property Manager in prioritizing maintenance and rehabilitation work.

? Oversee all physical improvements for properties such as roof replacements, appliance purchase, interior remodeling and energy improvements.

Master?s Degree and three years experience preferred.

Must have own transportation.

JOB DESCRIPTION

POSITION TITLE: Case Manager, Safe Places Residential Program

LOCATION: Northern Virginia, Fairfax County

REPORTS TO: Project Director, Homes for the Homeless

JOB SUMMARY:

Responsible for providing case management and support services to homeless victims of domestic violence participating in the transitional housing program. Ensure program success by creating and monitoring weekly goals and objectives that lead to self-reliance moving client families to permanent housing within 24 months. Maintain adequate records and reporting documents for caseload. Work closely with project director, property manager, and case manager.

MAJOR DUTIES & RESPONSIBILITIES:

? Conduct preliminary screening for potential clients.

? Schedule, attend, and perform intakes with potential participants eligible for the Safe Places Program.

? Provide direct services to a case load of ten (10) families participating in the Safe Places Residential Program

? Conduct weekly scheduled home visits/case management sessions at scattered-site housing units located throughout Fairfax County.

? Provide case management, guidance and support focused on trauma recovery to resident families for impacts related to domestic violence and other adjustment difficulties or personal barriers.

? Assess, model, monitor and refer life skills such as independent living, parenting, budgeting and home care, etc.

? Assist clients in obtaining needed services such as employment, childcare, health care, substance abuse treatment, mental health treatment and transportation, etc.

? Advocate with various systems, including but not limited to the police, schools, and courts.

? Complete monthly service updates, evaluation tools and other required reporting documents. Write weekly process notes and case summaries.

? Co-facilitate Monday Night children?s educational support groups.

? Recruit and screen volunteers for Monday Night Educational Support Groups and other special projects.

? Other assistance as needed to move families from homelessness towards self-reliance within 24 months.

QUALIFICATIONS:

? Must have own transportation.

? Available to work flexible day and evening hours

? Excellent oral and written communication skills

? Demonstrate the ability to work with minimum supervision

? Knowledge of domestic violence, post traumatic stress, child development

? 3 years experience working with families

? Knowledge of Virginia and Fairfax County resources

? BSW & 4 years experience working with victims of domestic violence preferred
